Usually, cartoon puzzle drawings use letters of the alphabeth as structural components, e.g. two O O for the eyes in a face. Numbers indicate the sequence of the letters in a word, hidden in the picture. Sometimes letters are difficult recognize but pointed out by a number. "Spell by number" means e.g. 1=B, 2=a, 3=r, 4=n, 5=a etc. "Spell clockwise" means in this case (0ct 17) that the letters, leading to the solution (the title of the picture) are arranged in a circle and the letters follow each other like the second-hand of your watch. Of course, you will have to find the beginning (trial and error) replacing the ? ? by the correct letter.
